AJOY ROYCHOUDHURY
68 yaer old Major A. suffered a myocardial
infarction in 2003 while he was driving a scooter. He understood the symptoms
and walked to the nearest hospital where they thromolysed him with
streptokinase. He was doing fine all these days. The angio showed triple vessel
disease,acteritic arteries and not suitable for angioplasty.
Major was fine and continued his daily chores and managed his part time job.
In 2005 Feb he reported for a regular check-up with stress thallium. The test went well as he had good exercise compliance. In the evening though he suffered another myocardial tighteness. he was again thrombolysed with urokinase this time. The ECG reverted to normal in 6 hours. His period of stay in the intensive cardio unit was uneventful. His ejection fraction was 60%. He was shifted to a private room in the hospital. After 7 days of total stay the doctors thought he was fit to be discharged as he was showing good recovery. walking on his own etc. suddenly in the morning while talking to his wife he gasped and closed his eyes. The doctor in the ward immediately came and tried all possible massages defibrillation, cathererization, pacing but nothing was of any use.
I am writing this because this man had a ejection fraction 60% showing a good
recovery. I don't know if the implantable defib would have saved him??? I think
its a time for all of us to think in that direction.
